This NYHealth interactive map offers regional and countywide information on the rates of diabetes and prediabetes throughout the State, as well as data on other factors that can provide context for this chronic condition’s impact on New Yorkers, including poverty rates, the number of adults remaining uninsured after health reform, and the number of primary care providers for residents by county.

In New York State, an estimated 1.8 million people have diabetes, a chronic condition that exacts an enormous human and financial toll. Another estimated 4.5 million New Yorkers have prediabetes, a condition that puts them at high risk for developing diabetes and its complications. To reverse the diabetes epidemic in New York State, NYHealth supports the spread of effective community-based prevention programs that reach people where they live, work and worship.

In addition to diabetes and prediabetes rates, the map has New York-specific information related to the National Diabetes Prevention Program (NDPP), a well-tested community-based program that has been shown to help reduce participants’ risk of developing the disease by 50%. The map shows the distribution of NYHealth grantees that are working to implement or grow the NDPP at community locations, as well as sites with NDPP coaches who have been certified by the Centers for Disease Control and Prevention to deliver the program and oversee a six-month maintenance period for participants. The map also shows sites in each county that have primary care providers who have achieved diabetes recognition. Providers who have attained diabetes recognition have demonstrated excellence in delivering diabetes care to patients.
