Health home models can improve the quality of care for those struggling with complex physical and mental health challenges while simultaneously containing costs. However, informing the implementation of health homes could be greatly enhance by drawing upon lessons learned from previous attempts at care coordination, particularly from the recent development and design efforts of HIV care systems.
The report summarizes the lessons learned from HIV case management models and offers recommendations for the development of health homes in New York State with a specific focus on patients with serious mental illness.
