This NYHealth-funded report, prepared by the Healthcare Association of New York State (HANYS), summarizes results from a survey of New York State health plans about the types of diabetes management programs each plan offers.

The survey also looked to determine the kinds of additional support the plans offer to providers for diabetes excellence and prevention programs, such as the National Committee for Quality Assurance (NCQA) and Bridges to Excellence (BTE) diabetes recognition programs, and patient-centered medical home (PCMH) recognition.

In 2012, HANYS surveyed major health plans licensed in New York State. The health plans were asked to provide information about available incentives or recognition for providers whose treatment of diabetes aligns with NCQA or BTE standards for diabetes or PCMH recognition. The health plans were also asked to list specific programs that offer support and/or address lifestyle and wellness issue.

The survey results include information on which plans offer diabetes disease management programs, financial incentives for diabetes or PCMH recognition, and reimbursement for providers’ diabetes self-management training.
