Diabetes is a largely preventable and manageable condition, and information and tools exist to help people manage the disease and reduce complications that lead to unnecessary hospitalizations and preventable death.

As part of an effort to reverse the diabetes epidemic, this grant supported primary care physicians in their efforts to provide excellent care for their patients with diabetes.

The Meeting the Mark: Achieving Excellence in Diabetes Care grant awarded $2,500 per physician who attained recognition from the National Committee for Quality Assurance (NCQA) or the Bridges to Excellence (BTE) diabetes program. The NCQA and BTE diabetes recognition programs are an indicator that clinicians are delivering the best care and achieving good outcomes for patients. This in turn helps to reduce the number of hospitalizations and emergency room visits for people with diabetes and to lower health care costs.
