In New York State, more than 4 million people are estimated to have prediabetes, a condition which heightens a person’s risk for developing diabetes and its complications.

To help ramp up diabetes prevention efforts across the State, NYHealth awarded grants to 25 community-based organizations to help them implement and spread the National Diabetes Prevention Program (NDPP) through the Foundation’s Laying the Groundwork initiative. In January 2014, NYHealth awarded Viridian Health Management (Viridian) a grant to provide technical support to the grantees. During the first six months of technical assistance (TA), Viridian offered broader support that focused on developing regional learning communities across sites. NYHealth awarded Viridian a follow-up grant to develop and offer a suite of in-depth TA services to the 25 Laying the Groundwork grantees.

Under this grant, Viridian offered a more tailored, in-depth set of TA services for each of the 25 organizations to help them sustain and grow their programs based on their needs and infrastructure. Specifically, Viridian served as a liaison between organizations delivering the program and third-party payers; assisted in the development of protocols and documentation templates for data collection and management to account for payments; and gathered and administered data from program sites to help them attain certification by the Centers for Disease Control and Prevention. It helped establish and broker relationships between health care providers and grantees to increase referrals of people with prediabetes into the program. To help grantees share best practices, it created an online learning community. Viridian also mentored NDPP instructors, known as lifestyle coaches, to ensure the best possible results for participants in the program.
