Although Nassau County on Long Island has a reputation as being one of the nation’s wealthiest suburbs, thousands of residents live in poverty.

Nassau Health Care Corporation’s (NuHealth) service area includes large tracts of designated medically underserved areas of the county populated by many low-income and vulnerable families. In 2008, census data estimated that more than 130,000 Nassau County residents were uninsured.

NYHealth awarded a grant to NuHealth to develop, implement, and provide a comprehensive medical home program, NuCare, for the uninsured in Nassau County.

Outcomes and Lessons Learned

  • Piloted NuCare at the Hempstead Health Center, enrolling 123 participants of which 73 received physicals;
  • Implemented NuCare at three additional health centers, enrolling and providing physicals to more than 4,000 participants by December 2012; and
  • Evaluated the NuCare program at two of the health centers, with survey findings indicating positive health trends for participants.
