Increased attention has been focused on population health—the idea of keeping entire communities healthy—in terms of achieving a quality and affordable health system.

In 2012, NYHealth awarded a grant to the National Academy of Sciences Institute of Medicine (IOM) to convene a population health roundtable. The roundtable, supported by NYHealth and other state and national funders, featured experts from multiple sectors and was charged with documenting the evidence behind population health and developing ideas to spur action in this area. The roundtable successfully raised awareness for possible interventions that could improve population health. In 2016, NYHealth awarded IOM a phase 2 grant for the roundtable to build on the momentum and recognition of population health and move the ideas generated from phase 1 into action.

Under this grant, IOM convened roundtable members who brought expertise from diverse sectors, including private businesses, non-health government agencies, health care delivery systems, academia, community organizations, and philanthropies. Roundtable members developed and disseminated ideas for concrete interventions that keep people healthy and reduce the need for expensive medical care. Specifically, IOM emphasized emerging interventions in federal, state, and community-level policies and activities. Over the course of this grant, roundtable members led workshops focusing on actions to accelerate community and state improvements in population health; provided administrative and technical support to collaboratives that emerged from the roundtable; and produced discussion papers and summary briefs on recommendations to move forward on initiatives and best practices to improve population health.
