In New York State, 20% of Medicaid beneficiaries drive almost 80% of total Medicaid spending. These patients often have multiple chronic conditions or disabilities and do not receive the coordinated care they require. In response, New York State undertook Chronic Illness Demonstration Projects (CIDP) targeted at high-need, high-cost, fee-for-service Medicaid beneficiaries with multiple chronic diseases. MDRC, in collaboration with the Department of Health, evaluated CIDP’s potential to generate sustainable and replicable models to reach thousands of Medicaid beneficiaries statewide.

This project determined whether the intervention changed patterns of health care use, reduced costs, and improved health outcomes. A companion implementation study by MDRC asssessed how the CIDP might have varied at different sites and how its implementation affected the outcomes. MDRC conducted a rigorous evaluation of New York State’s Chronic Illness Demonstration Projects (CIDP) to assess their effects on health care utilization, continuity of care, and costs.
