As the primary community component of NYHealth’s Diabetes Campaign, the Institute for Leadership’s (IFL) faith-based diabetes program, Defy Diabetes, has been successful in engaging faith-based leaders and securing participation of congregations located in communities with the highest risk for developing diabetes.

As part of a three-year statewide faith-based initiative on diabetes, the project has built the necessary infrastructure and momentum to continue to expand its reach to faith-based organizations throughout the State, and to increase these organizations’ capacity to lend their leadership and support to tackling the diabetes crisis.

IFL received an original grant amount of $513,244 from the Foundation for the third and final year of the initiative. NSYHealth awarded IFL an additional $10,000 in grant funding, bringing the new grant amount for year three to $523,244. This additional grant funding enabled IFL to contract a consultant who assisted the organization in preparing a grant application to the Centers for Disease Control and Prevention (CDC). The CDC grant application was intended to help IFL to secure Federal resources to further expand and replicate its current strategy to spread an effective diabetes self-management and prevention program.
