With more than 1.8 million New Yorkers suffering from diabetes and up to 4.2 million at risk for developing the disease, more trained providers and clinics are needed to improve the quality of care and quality of life for New Yorkers with diabetes.

The Healthcare Association of New York State (HANYS) has been NYHealth’s most effective Diabetes Campaign partner in assisting primary care providers with meeting the criteria for the National Committee for Quality Assurance (NCQA) or Bridges to Excellence (BTE) diabetes recognition. Achieving diabetes recognition is an indicator that clinicians are delivering the best care and achieving good outcomes for patients. With this grant, HANYS continued to build on the successes from the past two years by working with at least 125 hospital-owned clinics.

Under this grant, HANYS:

• Assisted an additional 600 providers in achieving NCQA or BTE diabetes recognition;
• Produced an annual report on the diabetes prevalence and diabetes admission and readmission rates in New York; and
• Provided technical assistance to members and share best practices and strategies with other Diabetes Campaign partners.

HANYS targeted late adopters of best practices in diabetes care and worked with them to improve their systems of care. To capitalize on financial incentives offered by Medicaid for outpatient departments to transform into Patient Centered Medical Homes (PCMHs), HANYS partnered with IPRO to provide hands-on technical support to help these sites adopt necessary system-level changes that will improve their coordination and quality of care, and meet PCMH certification criteria.
