Overview
Diabetes is a largely preventable and manageable condition, and information and tools exist to help people manage the disease and reduce complications that lead to unnecessary hospitalizations and preventable death. The Meeting the Mark: Achieving Excellence in Diabetes Care grant awards $2,500 per physician who attains recognition from the National Committee for Quality Assurance (NCQA) or the Bridges to Excellence (BTE) diabetes program. The NCQA and BTE diabetes recognition programs are an indicator that clinicians are delivering the best care and achieving good outcomes for patients.
