Cynthia Nassivera-Cordes, VP of Medical Support at Hudson Headwaters Health Network (HHHN) in Queensbury, New York, will provide an overview of how HHHN uses the patient portal in their patient-centered medical home initiatives. Included will be discussions on access and availability, test and referral tracking, and care management. This webinar is intended for clinicians, nurses, health educators and quality improvement specialists.
