		<description><![CDATA[<p>
	Founded as a settlement house in the neighborhood of Greenwich Village, Greenwich House has been responding to the vital needs of New Yorkers of all ages and backgrounds by providing critical social and health services, and cultural and educational programs since 1902. The Caring Community (TCC) is an almost 40-year-old organization that was founded to meet the needs of older New Yorkers residing in Greenwich Village and later extended its services to lower Manhattan. Together, Greenwich House and TCC share common missions, operate similar programs, and serve overlapping populations. In 2010, the nationwide economic crisis and projected municipal budget cuts placed the services delivered at TCC in serious jeopardy. To address these concerns, NYSHealth awarded Greenwich House a grant to evaluate and create a strategic alliance between Greenwich House and TCC. The intent of the alliance was to preserve vital services that might otherwise be lost as a result of the economic downturn and consolidate administrative overhead between both organizations.</p>

		<title>Taking Charge of Diabetes, a Collaborative Approach</title>

	In 2007, the New York State Health Foundation (NYSHealth) funded 12 grantees through the request for proposals (RFP), <em>Setting the Standard: Advancing Best Practices in Diabetes Management</em>. The goal of <em>Setting the Standard</em> was to move New York State&rsquo;s primary care system to adopt and spread best practices in disease management and establish them as the universal standard of care for patients with diabetes. According to the Family Health Network of Central New York (FHN), a multi-site, federally qualified health center in Cortland County, more people with diabetes in Cortland County die of complications related to diabetes than in other parts of the State. Data collected by FHN suggested that a target population existed that may not have had a consistent medical home. With funding from NYSHealth, FHN worked to expand its existing diabetes program that was launched in 2006 to include more care teams and locations in Cortland County.</p>

	To address the diabetes crisis in New York State, the New York State Health Foundation (NYSHealth) invested in a five-year, $35 million Diabetes Campaign with the goal of reversing the epidemic. Under its strategy to improve clinical care and patient outcomes, the Foundation expects to have 3,000 of New York&rsquo;s primary care physicians providing excellent care to patients with diabetes, as evidenced by achieving certification from national diabetes recognition programs. The National Committee for Quality Assurance (NCQA) is a nationally renowned diabetes recognition program that recognizes physicians who meet the highest standards of diabetes control. In 2010, NYSHealth awarded a grant to NCQA to help underwrite application, data collection tool, and administrative fees for physicians and physician practices interested in attaining diabetes recognition through the Campaign, as these fees can be a major barrier for some to apply.</p>

	In the Black Rock region of Buffalo, access to mental health care is limited. In 2008, the average patient wait time was six weeks to see a counselor and more than three months to see a psychiatrist. The Neighborhood Health Center (NHC), a federally qualified health center in the Black Rock region, serves more than 10,000 patients annually. Established more than 20 years ago, it provides care and services in pediatrics; internal medicine; obstetrics and gynecology; dentistry; and nutrition education to the uninsured and underinsured. To address the unmet mental health needs of residents in this underserved area, the New York State Health Foundation (NYSHealth) awarded a grant to NHC to increase patient access to on-site mental health services.</p>

	The Healthcare Association of New York State (HANYS) represents an important cohort of primary care providers in New York. Of the 206 hospitals that are part of HANYS, there are more than 160 hospitals with ambulatory clinics that receive more than 11.3 million clinic visits a year. In addition, several of the outpatient departments report that more than 70% of their patient population is covered by Medicaid. In 2008, the New York State Health Foundation (NYSHealth) launched its NYSHealth Diabetes Campaign in response to the growing epidemic of diabetes across the State. To advance the Campaign among hospital outpatient clinics, NYSHealth made a grant to HANYS to engage its network of outpatient health clinics, and provide them with technical assistance to adopt a new model of care to improve the care and outcomes of patients with diabetes.</p>
