Kate Ebersole, Principal, KEE Concepts Consulting, has been working in health care reform and community health improvement for the past 10 years. She previously spent 20 years in quality systems, sales, and engineering in the automotive supply manufacturing industry before opening her own coaching, training, facilitation, and consulting business in 2003. After working several years as a team coach with primary care practices, helping them prepare their practice teams for patient-centered medical homes (PCMHs) and other quality initiatives, Ms. Ebersole joined the P2 Collaborative of Western New York in 2010. Within the P2 environment, she oversaw an eight-county network of quality improvement coaches supporting PCMH, technology upgrades, meaningful use, and NYSHealth’s Meeting the Mark initiative for diabetes recognition in primary care offices. Ms. Ebersole also developed and implemented care coordination/community health worker pilots in Buffalo, Niagara Falls, and Pembroke; facilitated the State’s Community Health Improvement Plan/Community Service Plan development process in six of the eight counties in Western New York; and managed various aspects of the Robert Wood Johnson Foundation’s Aligning Forces for Quality initiative, including consumer engagement, Medicaid quality improvement, and expansion of care coordination pilots. Since 2014, Ms. Ebersole has been an independent consultant working across New York State. Some of her projects during this time include being the facilitator for the Evidence Based Intervention Learning Collaborative through the New York Academy of Medicine and the Advancing Prevention Project (which works with health departments across the State). She has also worked as the local improvement advisor for Cattaraugus County through a SCALE grant (an Institute for Healthcare Improvement community health improvement initiative) as well as provided coaching and facilitation in support of the Creating Healthy Schools and Communities grant in Chautauqua County. Ms. Ebersole also has acted as an outreach agent for the Finger Lakes Performing Provider System for its population health roadmap deliverable under the Delivery System Reform Incentive Payment (DSRIP) program.
